You can say that in America before Obamacare goes fully into force (2014 unless repealed) the system is one of health care being a PRIVILEGE based on the ability to afford it or get a job in which covers you. But actually one could say that many people who have the privilege do also have a RIGHT to health care by virtue of them or their employer having paid into an insurance pool. Its a legal right under contract. Those having retired have a RIGHT to Medicare by virtue of their having paid their Medicare taxes. And that right is a right in LAW. Same with Medicaid eligibility.

The real issue is that there are 45 million Americans who have no insurance because they have lost their job; because they have become ill and have had to quit their job, or simply cannot afford the premiums because most U.S. states, unlike many other countries, allows insurers to cherry pick who they insure and on what terms. So the issue is really DO WE WANT THE SICK, THE POOR, AND THE UNEMPLOYED not to be able to afford to insure themselves and thus become the UNDERprivileged (perhaps the UNPRIVILEGED).

Most other countries ensure that RIGHTS are tied to OBLIGATIONS. You have the RIGHT TO HEALTH CARE but you also have the OBLIGATION to pay your taxes and contribute to your health care costs over your lifetime.  It was conservative policy at one time that everyone should be insured and should contribute to the cost of their care when they are able to do so. And yes the mandate WAS conservative policy at one time. Its what we proposed in opposition to Hillarycare. And that is good. Quitting or losing a job in France or Germany or Canada does not leave you without health care and this means that in those countries there is better labor flexibility.

So yes, health care CAN BE A RIGHT if we want it to be a right. But it comes with certain OBLIGATIONS. That is the basis of health care rights in Europe, Canada, Massachusetts, and in 2014 across the whole of the United States.  If Americans choose to repeal Obamacare they will be going back to it being a right of privilege.  There is no right or wrong answer to the question. It what we want it to be that really matters.]]></description>

		<description><![CDATA[The Express Scripts- Medco merger is bad for consumers and community pharmacy. I have run my family owned pharmacies for the past 41 years. These companies do not value the services we provide for our patients and their payment structure leaves little if any profit to add services that would improve patient care. I serve an inner city area where my two pharmacies are the only pharmacies. Express Scripts serves the two largest medicaid HMO. When Express Scripts was awarded the contract, there payment structure increased my account receivables by at least 20 percent. I was forced to close another location. These companies are unregulated in most states and have denied me payment exercising professional judgment that the Commonwealth of Virginia grants me.

Everyday I improve the quality of life for my patients. I provide services above and beyond the standards of care. Express Scripts pays me the cost of the medication plus $1,60 for most prescriptions( One HMO saw the value of my services and they had Express Scripts pay me more). The $1.60 is supposed to cover containers, labels, internet fee( Yes I have to pay every time I submit a prescription), insurance, rent, employees) Express Script nets over 3,00 on each prescription it processes. How is this good for free enterprise.

On the consumer side, in too many instances I have had patients denied care without a review by an MD. I had another patient who was locked into a Walgreen&#039;s pharmacy and when the patient wanted to switch to me, the patient was denied services.

As Americans we take too much medication. And the push to fill more prescriptions to maintain the bottom line will end up costly all of us in the long run.

I hope this merger is blocked and that all of the PBMs will look at what they are doing not only to a great profession but our patients each and every day.
